Responsibilities

Job Summary

The Patient Navigator position, under the direction of the Lead Patient Navigator, is responsible for the care coordination to future, current, and past patients referred to the Center of Excellence for Opioid Use Disorder. Facilitates engagement and reengagement in substance use disorder treatment and aids in coordinating appointments or services for additional medical, behavioral, and social needs. In addition, the patient navigator would maintain a clinical caseload of patients and accurately input patient data into an electronic tracking system.

Essential Functions
  • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
  • Communicates with referral sources (i.e., hospitals, inpatient rehab, or IOP programs) and assists with scheduling admission/intake appointments for medication for opioid use disorder.
  • Assess the physical, behavioral, or social health needs and barriers through semi-structured interviews and outreach efforts.
  • Assist patients in obtaining services or appointments to meet those physical, behavioral, or social health needs.
  • Assess patient engagement in medication for opioid use disorder treatment and help to retain patients in treatment
  • Keep current local, state, and federal regulations regarding confidentiality of patient information. Maintain clinical records that are in compliance with regulatory guidelines.
  • Maintain accurate record keeping of patient care, progress, and concerns by completing clinical documentation in a timely manner.
  • Accurately collect and input patient data into an electronic tracking system.
  • Collaborate on shared cases with other clinicians on the treatment team, including the Medical staff, Counselors, Recovery Specialist, and Supervisors.

Minimum Education and Experience Requirements:

Education:

Bachelor's Degree in psychology, social work, or another human services field required. Master's Degree is preferred.

AND

Experience:

At least 1 year of experience working in a behavioral health or medical health care organization with a focus/concentration on substance use disorders (particularly opioid use disorders).

Conditions of Employment

Since this position will require the selected candidate to engage in regular contact with children in the form of care, guidance, supervision or training, you will be required to meet the requirements of the Child Protective Services Act, including the Pennsylvania State Police criminal background check, the Pennsylvania Department of Welfare child abuse criminal background clearance, and an FBI finger print based federal criminal records check. The selected candidate will not be allowed to commence working until all three (3) are completed satisfactorily.
