Responsibilities

Thomas Jefferson University Hospital is hiring a Full Time Heart Failure Coordinator!

Job Summary

Oversees clinical care of patients admitted with heart failure at TJUH and JMH; collaborates with appropriate clinical staff to ensure optimal HF care at TJUH and JMH. Ensures Heart Failure education for patient and family. Ensures an appropriate HF discharge plan is developed and includes follow up with MD appointment, Home care, HF outpatient center when indicated. Maintain data on heart failure outcomes - Guideline directed medical therapy, LOS and readmissions, HF resource utilization and quality indicators.

Essential Functions
  • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
  • Provides consultation, education and support to patients and patient caregivers related to care giving responsibilities
  • Maintains effective working relationship with medical team and all agency personnel providing care to the patient.
  • Prepares and submits timely and accurate statistical reports on HF related metrics and transitional care interventions.
  • Maintains effective collaborative communication with multidisciplinary team members in maintaining quality care for the patient.
  • Attends educational activities and departmental meetings as required.
  • Documents all care management activities for each patient/client.
  • Other duties as assigned


Minimum Education and Requirments:
  • BLS & ACLS
  • BSN
  • 3 years of Cardiac Exp.
    • Heart Failure Preferred


Conditions of Employment

Covid Vaccination is a requirement for employment at Jefferson for employees working at Jefferson's clinical entities or at the University.
